## New Patient Process

#### Are appointments in person or virtual?

Our services are provided through telehealth. Patients attend appointments virtually
using a secure platform.

#### Do you offer services in English and Spanish?

Yes. Alquimedez Mental Health Counseling offers bilingual services in English and
Spanish.

#### What happens after I submit the intake form?

After your intake form is submitted, our office will review your information and
may contact you by phone, SMS, email, or WhatsApp to confirm details, request missing
documents, verify insurance, and schedule your appointment.

#### How do I become a new patient?

To become a new patient, please complete our online intake or contact our office.
Our team will review your information, confirm service availability, verify insurance
when applicable, and help you schedule the appropriate appointment.

## **Insurance and Payment**

#### Am I responsible for deductibles, copays, or coinsurance?

You are responsible only for amounts properly assigned to you under applicable law
and your insurance plan, such as permitted copayments, deductibles, and coinsurance.
An insurance denial does not automatically make the denied amount your responsibility.
We review denials, payer requirements, and applicable protections before billing
you.

Noncovered or out-of-network services will be discussed in advance when reasonably
possible, and any required notice or separate agreement will be obtained. Read our
[Financial Responsibility Agreement](https://www.alquimedez.com/financial-responsibility-agreement).

#### Do you accept private-paying patients?

Yes. Patients who are uninsured or choose not to use insurance may request self-
pay options. Before you agree to a self-pay or separately priced service, we will
explain the fee and payment terms. A Good Faith Estimate will be provided when required
by federal law. Read our [Financial Responsibility Agreement](https://www.alquimedez.com/financial-responsibility-agreement).

#### How do I know if my insurance covers services?

Our office can help verify your benefits. Verification, prior authorization, and
quoted copayments are not guarantees of payment. Coverage may depend on your plan,
provider, service, location, and date of service.

Please provide current insurance information and notify us of changes. You may also
contact your insurer to understand your benefits. Read our [Financial Responsibility Agreement](https://www.alquimedez.com/financial-responsibility-agreement).

#### Do you accept insurance?

Yes. We work with several insurance plans. Coverage depends on your specific plan,
state, service type, deductible, copay, coinsurance, and authorization requirements.

## **Therapy**

#### Can therapy be done by telehealth?

Yes. Therapy sessions are provided virtually through telehealth.

#### How often will I attend therapy?

Frequency depends on clinical needs, treatment goals, provider recommendation, and
patient availability. Some patients attend weekly, while others may attend biweekly
or as clinically appropriate.

#### What therapy services do you provide?

We provide individual therapy for concerns such as anxiety, depression, trauma,
stress, life adjustment issues, emotional regulation, relationship concerns, grief,
and other behavioral health needs.

## **Medication Management**

#### Can prescriptions be sent electronically?

When clinically appropriate, prescriptions may be sent electronically to the patient’s
preferred pharmacy.

#### Do I need therapy if I receive medication management?

Some patients benefit from both therapy and medication management. Your provider
may recommend therapy, medication management, or both depending on your clinical
presentation.

#### What is medication management?

Medication management involves psychiatric assessment, review of symptoms, medication
monitoring, discussion of side effects, treatment response, and follow-up care with
a prescribing provider when clinically appropriate.

## **Psychiatry**

#### Is psychiatry available by telehealth?

Yes. Psychiatry and medication management services are offered virtually when clinically
appropriate and legally permitted.

#### What conditions may psychiatry help with?

Psychiatric services may support patients experiencing symptoms related to anxiety,
depression, mood disorders, trauma-related symptoms, ADHD-related concerns, sleep
difficulties, and other behavioral health conditions.

#### Do you provide psychiatric evaluations?

Yes. Psychiatric evaluations may be available for patients who need assessment for
mental health symptoms, diagnosis, and possible medication treatment.

## **Immigration Psychological Evaluations**

#### Do I need an attorney before requesting an immigration evaluation?

An attorney is strongly recommended for immigration legal matters. The psychological
evaluation is a clinical document and does not replace legal advice.

#### What documents are needed for an immigration evaluation?

Helpful documents may include identification, immigration-related notices, attorney
referral information, personal declarations, police reports, medical records, prior
therapy records, protection orders, or other supporting documents. The exact documents
depend on the type of case.

#### What does an immigration evaluation include?

An immigration psychological evaluation may include a clinical interview, review
of psychosocial history, trauma history, emotional symptoms, family impact, and
preparation of a written report when clinically appropriate.

#### Do you provide immigration psychological evaluations?

Yes. We provide psychological evaluations for immigration-related matters, including
cases involving VAWA, U-Visa, T-Visa, political asylum, extreme hardship, and related
immigration concerns.

## **Impaired Driving and SAP Assessments**

#### What should I bring for an impaired driving or SAP assessment?

You may need identification, court documents, DMV or DOT-related letters, prior
treatment records, toxicology reports, completion certificates, and any referral
paperwork.

#### Do you provide Substance Abuse Professional assessments?

Yes. Substance Abuse Professional services may be available for individuals who
require a SAP-related process.

#### Do you provide DUI, DWI, DWAI, or impaired driving assessments?

Yes. We provide impaired driving-related evaluations and assessments when appropriate,
including services related to DUI, DWI, DWAI, and impaired driving offenses.

## **Telehealth Requirements**

#### Can I attend telehealth from any state?

Telehealth availability depends on the state where the patient is physically located
at the time of service and the provider’s licensure. Our office will confirm whether
services can be provided based on your location.

#### Can I attend a telehealth appointment from my car or workplace?

You should attend from a private and safe location where confidentiality can be
maintained. Your provider may determine whether the setting is appropriate for the
appointment.

#### What do I need for a telehealth appointment?

You need a private location, reliable internet connection, a smartphone, tablet,
or computer with camera and microphone, and access to the telehealth link provided
by the office.

## **Records Needed**

#### Why are records important?

Records help the provider understand your history, coordinate care, support diagnostic
accuracy, and prepare appropriate documentation when needed.

#### How can I send records to the office?

Records may be submitted through secure office-approved methods, such as patient
portal upload, fax, secure email, or other approved communication channels.

#### What records should I provide before my appointment?

Depending on the service, records may include prior mental health records, medication
lists, hospital discharge summaries, school records, legal documents, immigration
documents, medical records, insurance information, identification, or referral forms.

## **Communication by SMS and WhatsApp**

#### Is SMS or WhatsApp private?

SMS and WhatsApp may have privacy limitations. Sensitive clinical information should
be shared through secure, office-approved channels whenever possible.

#### Should I use SMS or WhatsApp for emergencies?

No. SMS and WhatsApp should not be used for emergencies, crisis situations, or urgent
clinical concerns.

#### Can I communicate with the office by SMS or WhatsApp?

Yes. Our office may use SMS or WhatsApp for appointment coordination, reminders,
document requests, and general administrative communication.

## **Emergency Policy**

#### What if I have suicidal thoughts?

Tell your provider immediately during a session. If you are in immediate danger
or may harm yourself or someone else, call 911, go to the nearest emergency room,
or contact 988.

#### Can Alquimedez provide emergency services?

No. Alquimedez Mental Health Counseling is not an emergency crisis service. Telehealth
appointments and office communication are not a substitute for emergency care.

#### What should I do if I am having a mental health emergency?

If you are experiencing a life-threatening emergency, call 911 or go to the nearest
emergency room. If you are experiencing a suicidal crisis or emotional distress
in the United States, call or text 988 for the Suicide & Crisis Lifeline.

## **Cancellations and No-Shows**

#### What happens if I miss my appointment?

Contact the office to discuss rescheduling. Any no-show fee is subject to the exceptions,
applicable law, and insurance restrictions in our [Financial Responsibility Agreement](https://www.alquimedez.com/financial-responsibility-agreement).
You may request review of a disputed charge before collection.

#### Is there a cancellation or no-show fee?

A $25.00 no-show fee may apply if you do not attend a scheduled appointment and
have not canceled it before its scheduled start time, only when permitted by law
and your insurance requirements and after you have received the policy. Our agreement
does not establish a separate late-cancellation fee, and the no-show fee is not
billed to insurance as a clinical service.

No fee is assessed for provider cancellations, duplicate or incorrectly recorded
appointments, or verified practice-related technical failures. Emergencies, hospitalization,
disability-related access barriers, and other circumstances may be reviewed individually.
Medicaid and other payer restrictions apply. You may request review of a disputed
fee. Read our [Financial Responsibility Agreement](https://www.alquimedez.com/financial-responsibility-agreement).

#### How do I cancel or reschedule an appointment?

Please cancel or reschedule at least 24 hours before your appointment through an
approved Alquimedez scheduling channel. Call [315-505-2400](https://www.alquimedez.com/3155052400)
for assistance. Read our [Financial Responsibility Agreement](https://www.alquimedez.com/financial-responsibility-agreement)
for the cancellation and missed-appointment terms.

## **Patient Portal Access**

#### Can I complete intake paperwork through the portal?

Yes, when available. Completing paperwork before your appointment helps avoid delays.

#### What if I cannot access the portal?

Contact the office for assistance. Staff can help confirm your email, resend access
instructions, or guide you through the next steps.

#### Do patients have access to a patient portal?

Yes. Patients may receive access to a patient portal for forms, documents, appointment
information, and communication depending on the system being used.

## **Remote Patient Monitoring for Mental Health**

#### How do I get started with RPM?

Patients interested in RPM should contact the office. The team will review eligibility,
clinical appropriateness, insurance considerations, and next steps.

#### Is RPM covered by insurance?

and documentation requirements. Our office can help review potential coverage when
applicable.

#### What may RPM monitor?

Depending on the program and clinical need, RPM may involve monitoring health indicators
such as blood pressure, weight, glucose, sleep patterns, or other relevant health
data.

#### Is RPM a core service at Alquimedez?

RPM may be offered when clinically appropriate and available. Patients should contact
the office to confirm whether RPM is currently available for their needs.

#### What is Remote Patient Monitoring?

Remote Patient Monitoring, also known as RPM, uses digital tools or devices to help
monitor certain health information outside of a traditional office visit.
