Impaired Driver Services

An impaired-driving referral may involve an assessment, education, treatment, or other requirements set by a court, licensing agency, employer, or referring program. These requirements vary, so the purpose of your request matters.

Before scheduling

  • Identify the organization requesting the assessment and the state involved.
  • Check the exact service, approved-provider requirements, and submission deadline.
  • Ask whether a specific form or report is required.
  • Contact our office to confirm whether your request can be accommodated.

Assessment and next steps

An assessment and any subsequent treatment are different services. A referral does not by itself determine a diagnosis or treatment recommendation. Completing an appointment does not guarantee license reinstatement, a particular legal outcome, or acceptance of a report.

Review our court-related assessment information. For interpretation of a legal requirement, speak with your attorney or the organization that issued it.

Request an appointment

Request an Appointment or contact our office to discuss your request. Tell our team your state and the type of support you are seeking so availability and next steps can be confirmed. Submitting a request does not confirm an appointment.
